This event will have a bouncy slide, a 3-in-1 sports bounce item, and a bouncy obstacle course!  DJ Danny Roselle will be spinning tunes.

Requirements for Student Participation

• Sign-up: Use this link to sign up by June 10 (suggested donation $20 per family).  L-H Signup/Donation Form
• Permission form: Parents/guardians must print and submit by June 10 a completed permission/waiver form to their child's teacher for every student that wishes to participate. L-H End of School Celebration Permission/Waiver Form
• Attend event and supervise:  A parent/guardian must accompany the student on the day of the event to obtain a wristband and supervise their child during the event.  

Requirement for Dunk Booth Participation (5th Graders Only)

• If your 5th grader wants to sit in the dunk tank, you must print and submit by June 10 a completed permission/waiver form to your child's teacher.  L-H Dunk Tank Permission/Waiver Form

Only printed versions of forms will be accepted.  Please submit all forms by Tuesday, June 10

Volunteers

This event is super fun for everyone, but we need volunteers to make is a success!  Please sign up here.  We still need volunteers from classes 2P, 3B, 4C, and 4F to hand out bracelets.  We also need help monitoring the bouncy houses and cleaning up.
