Lincoln-Hubbard Volunteer Documents & Forms

Thank you for volunteering!

Below is the necessary paperwork to be completed by Summit Volunteers "supervising students" as required by the State and the Summit Board of Education.  This is a one-time verification process.

1(A). If you are a Lincoln Hubbard Parent Volunteer,
            click on Fingerprinting Procedures
            Please make sure to classify your job category as that of                    VOLUNTEER.

1(B).  If you are an After School Personnel and is facilitating (being              paid) or volunteering at a PTO funded event after school                    hours, click on  After School Fingerpinting Procedure.
          Please make sure to classify your occupation as that of                      OTHER.

2.  MorphoTrust form with Receipt
       Once you have been fingerprinted, bring your                                     MorphoTrust form with receipt back to the Board Office.

3.  Criminal History Record Check
        You must provide the Board Office with a copy of your                        confirmation of electronic payment for Criminal History                      Record Check.

4.  Criminal History Clearance Letter
        Within 2-3 weeks of your fingerprints, you may go back to         the Criminal History Review website and print your criminal         history clearance letter.  The state no longer sends these         via US mail.  The Board Office does not receive a copy.          Please provide a clear hard copy of this letter (showing the         center seal) to the Board Office.

5.  District Emergency Form

The above paperwork must be completed prior to volunteering.

Did You Know....

to drive a car-load of kids from your child's class on a School Field Trip, or even just to chaperone a Class Field Trip, you MUST be approved by BOTH Summit Board of Education AND Lincoln-Hubbard Elementary School?

​1.  Fingerprinting and a background check is required for all                       volunteers.  Follow the Board of Education Clearance                           Procedures as shown on this page.  Once you are given                     approval from Valerie Bampe at the Board Office, inform Alicia        Downey at Lincoln Hubbard Elementary School. (One-time only           process.  Only L-H Main Office has access to this confidential             information.)

2.  Submit the completed Driver Volunteer Form, a copy of         your Insurance Declaration Page, Proof of Insurance and copy
        of current Driver's License to Alicia Downey at Lincoln Hubbard         Elementary School.  (To be submitted yearly when providing         transportation.)

The above paperwork must be completed by all volunteers who are providing LHES student transportation and/or supervision.
